Health and Developmental History: Interval Health and Developmental History

Once the initial history has been obtained, providers are required to update the health and developmental history at each subsequent preventive health visit. This interval history should summarize events affecting the child’s or teen’s health since the last visit.

When obtaining an interval history, ask the child’s family about events or changes that may have occurred since the last visit. Make note of the following:

  • Recent illness or injury
  • Visits to other health care providers or facilities
  • Emergency room visits
  • Hospitalizations
  • Chronic health problems or conditions
  • Medications
  • Immunizations
  • Allergies
  • Developmental or behavioral concerns
  • Changes in health of family members
  • Stresses in the home (move, loss of income, divorce)

The information gathered during the interval history helps to complete the child's entire health history over time.

 

Adolescent Health and Developmental History

In addition to the other components of a health and developmental history, adolescent health histories should also include the following:

  • Psychosocial assessment (family and friends)
  • Education/school performance
  • Emotional/behavioral status
  • Eating behaviors, physical activity
  • Activities and interests
  • History of substance abuse and personal violence
  • Sexual maturation
  • History of sexual activity and use of contraceptives
  • Menstrual history (for females)
  • Record and documentation of past immunizations
