Resources for Cross-Sector Collaborative Care
In May 2018, the DC Collaborative for Mental Health in Pediatric Primary Care kicked off a learning collaborative focused on cross-sector early childhood mental health. This project aimed to improve effective and collaborative referral systems between primary care, early childhood mental health, and early childhood education providers across the District to increase timely and coordinated access to behavioral health services for children under age five.
One of the resources developed was the handout, Supporting Your Child’s Development which is an educational handout for families focused on increasing knowledge and understanding of the developmental/behavioral referral process. This document can be used in all three sectors—primary care, early childhood education, early childhood mental health—when referring for developmental and behavioral concerns. This resource is available in English and Spanish.
